All staff are responsible to self-monitor DAILY for COVID-19 symptoms via the school
app prior to attending work. Staff with symptoms must stay at home and report to their

  • If symptoms appear after reporting to school or work, individuals will be sent home.
  • Staff will have to notify HR.

Reopening Plan

Canutillo ISD values the health, safety, and welfare of our students, faculty and staff above everything else. We commit to continuously revise current public health guidelines by national, state and local health authorities to provide the best protocols for a safe back to school in-person plan to welcome all our students.

Here are some key points from our new CISD Opening Safety Protocols for the upcoming Fall 2021.

COVID-19 Vaccine Questionnaire for all district personnel

The Human Resources Division continues monitoring the COVID-19 Vaccine progression for district employees and is requiring that all employees submit a COVID-19 Vaccine Questionnaire through the link below; if you do not wish to disclose information, you may elect the choice, "Do not wish to disclose" *however, all must complete a questionnaire.

If you have submitted your record -- thank you, no action is needed; disregard this message.

If you are submitting this questionnaire for the first time or need to update your previous response, please follow the instructions below:

  1. Sign out of all Gmail Accounts
  2. Sign into your Canutillo ISD email; All Employees will be required to sign into their district email account in order to submit any information.
  3. Update your record

 No need to isolate or report case (if asymptomatic). Review immunization card.   Initiate COVID-19 Protocol
    Report case to HR and District PHEM.* Collect Exposures Information 
    To return to work:
HR will send back a clearance letter when the employee or exposures may return to work. 

Employee must complete back to work screening tool 
     * HR will require a doctor's note for clearance if employee is out for more than 3 days. District RN/PHEM will manage positive lab reports and exposures of staff.
