Transition Services

Under the Individuals with Disabilities Education Act (IDEA), transition services are defined as a coordinated set of activities within a results-oriented process designed to improve the academic and functional achievement of students with disabilities. These services are intended to facilitate a successful transition from high school to postsecondary education, vocational education, integrated or supported employment, continuing and adult education, adult services, independent living, and community participation.

Transition planning is individualized and based on each student's strengths, preferences, interests, and needs. The Admission, Review, and Dismissal (ARD) Committee also considers appropriate interagency responsibilities and community linkages to support the student's transition to adult life.

The resources listed below provide information about agencies and services that may assist students and families as they prepare for life after high school.
