The Human Resources Division continues monitoring the COVID-19 Vaccine progression for district employees and is requiring that all employees submit a COVID-19 Vaccine Questionnaire through the link below; if you do not wish to disclose information, you may elect the choice, "Do not wish to disclose" *however, all must complete a questionnaire.
If you have submitted your record -- thank you, no action is needed; disregard this message.
If you are submitting this questionnaire for the first time or need to update your previous response, please follow the instructions below:
- Sign out of all Gmail Accounts
- Sign into your Canutillo ISD email; All Employees will be required to sign into their district email account in order to submit any information.
